Nidan Panchak
- Nidana
- Ativyayama, atibhara vahana, ageing, obesity, ruksha-sheeta ahara, vega dharana, prolonged squatting and stair use, previous joint injury.
- Purvarupa
- Sandhi sphutana, mild stiffness after rest, fatigue in the thighs after walking.
- Rupa
- Sandhi shoola on movement, atopa, vata-purna-driti sparsha shotha, akunchana-prasarana vedana, and reduced walking distance.
- Samprapti
- Vata prakopa → Shleshaka Kapha kshaya in sandhi → Asthi-sandhi shoshana → sphutana and shoola; when Ama is present, Margavarodha adds shotha and stambha.
- Sadhya-asadhyata
- Krichhra sadhya in early cases; yapya once deformity, joint-space collapse or varus is established.
Decide the type before prescribing
- Ama-pradhan: swelling, warmth, stambha, coated tongue — start with Ama pachana and rukshana, delay oil massage.
- Kshaya-pradhan: dry crackling, night pain, no swelling — start with Snehana, Brimhana and Bala-Ashwagandha line.
- Weight, quadriceps power and walking distance are recorded at every visit — these predict outcome more than any drug change.
Chikitsa Sutra
Ama pachana → Vatanulomana → Snehana with Sthanika chikitsa → Brimhana and Asthi poshana → maintenance with exercise. Panchakarma is planned into the schedule, not offered as an afterthought.
- Week 1–2: Ama pachana with Rasna-Guduchi line, Vatanulomana with Eranda-based drug.
- Week 2–6: Janu basti with Mahanarayana or Kottamchukkadi taila, 30 minutes daily for 7–14 days.
- Patra pinda sweda or Valuka sweda for stambha; Upanaha for chronic dry joints.
- Week 6 onwards: Asthi-poshaka Rasayana — Laksha, Asthishrinkhala, Praval, cow's ghee with milk.
- Shodhana: Mridu Virechana followed by Yapana or Tikta ksheera basti in chronic cases.
Exercise prescription that patients actually follow
- Static quadriceps and straight-leg raise, 10 repetitions three times daily — start on day one.
- Avoid deep squats, cross-legged sitting for long periods, and stair climbing during acute pain.
- Walking on level ground, gradually increased; cycling on a stationary cycle without resistance.
- Weight reduction target of 5–7% is discussed openly in obese patients.
Formulations, dose and anupana
Doses are for an average adult and are adjusted to Agni, Bala, Vaya and associated conditions. Pregnancy, lactation, renal or hepatic compromise need individual revision.

Pathya
- Warm, unctuous, easily digestible food; ghee with milk
- Til, methi, garlic, drumstick, Bala and Ashwagandha in diet
- Regular sleep, warm water for bathing, use of a Western toilet
- Daily prescribed knee exercises and weight control
Apathya
- Ruksha, sheeta, vasi food and excess raw salad in cold weather
- Prolonged squatting, cross-legged sitting, stair climbing during flares
- Suppressing natural urges, night vigil, exposure to cold wind and AC draft on the knee
- Complete inactivity — immobility worsens Sandhivata as much as overuse
Red flags — refer immediately
- Single hot, red, exquisitely tender joint with fever — suspect septic arthritis, refer same day
- Locking, giving way, or true joint instability — orthopaedic and imaging referral
- Morning stiffness over one hour with small-joint symmetrical involvement — evaluate for rheumatoid arthritis
- Neurological deficit, foot drop, bladder or bowel involvement — spinal cause, urgent referral
- Grade 4 changes with deformity and night pain at rest — discuss surgical options honestly
Frequently asked
- Is Janu basti useful in knee osteoarthritis?
- Yes, in Vata-kshaya predominant knees it reduces pain and stiffness noticeably within a 7–14 day course. It is postponed when the joint is hot and swollen with Ama.
- How long should Sandhivata treatment continue?
- Oral treatment usually runs 3 months with a Panchakarma module in between, after which maintenance depends on exercise, weight and diet rather than continuous medication.
- Can Sandhivata be reversed?
- Early cases improve substantially in pain and function. Once joint space is markedly reduced the goal shifts to yapya management — controlling pain, protecting function and delaying surgery.
